**Due to minimal manning on Fridays, not all services will be available (e.g. fingerprint override). These services will require a visit during normal business hours Monday - Thursday.**

Schedule ONE appointment for every TWO people requiring a new ID for all locations except the Air National Guard in Salt Lake City. They require ONE appointment for EACH request for support

First and last name
The number of dependents who are needing ID cards
Phone numbers where applicants can be reached.
*Cell phones are preferred in order to notify the applicant if the system goes down on short notice.*

Two forms of ID (Identifications must be current).

Acceptable Documents

All dependents must have an 1172-2 signed by the sponsor. If the sponsor is NOT able to physically appear with the dependent requesting the ID the sponsor can: sign the DD1172-2 with a notary OR Click the "Make an Appointment" button below and select the Family ID Cards option to sign the required DD1172-2. This form is only good for 90 days.

* Note - It takes up to 24 hours for TRICARE to see a new family member added to DEERS.*

Documentation of changes or updates

Adding a spouse requires:
- Marriage Certificate
- Birth Certificate
- Driver's License
- SSN Card

Removing a spouse requires:
- Divorce Decree

Adding a dependent (child) requires:
- Birth Certificate
- SSN Card (optional but will only be added with a temporary ID number is not provided)

Military service members and their family members
Retired service members and their family members
100% disabled veterans and their family members
Medically retired service members their family members
Qualifying former spouses/unremarried former spouses and their family members
Qualifying federal employees
Qualifying retired federal employees
Qualifying state employees
Qualifying federal contractors

*Note - We do NOT issue VA cards at any of our locations*. Instructions for this process can be found here: https://www.va.gov/records/get-veteran-id-cards/vic/

*Note - All documents must be originals or certified copies and physically present. If it is a foreign document, it must be translated into English.

*Note - All identification documents must be current (i.e. NOT expired)*

Adults - All persons 18 or older wishing to receive a Military USID and/or CAC, must bring their old CAC or USID card and two forms of a current state or federally issued ID. Types of acceptable ID's can be found here.
Retired Military - If this is the first visit to a DEERS facility since retiring, member must bring a copy of their retirement order, 20-year retirement letter, and DD214 or NGB 22. Retired personnel must bring their old retired USID card AND two forms of current government issued ID.
100% Disabled Veteran - If this is the first visit to a DEERS facility since receiving disability status, must bring a letter from the VA stating entitlement for 100% disabled along with their DD214. 100% disabled veterans must bring their old USID card AND two forms of current government issued ID.
Medically Retired Service Members - If this is the first visit to a DEERS facility since receiving a medical retirement, member must bring their medical retirement orders and DD214 or NGB22. Medically retired service members must also bring their old USID or CAC, and two forms of current government issued ID.
Family Members - Must have an 1172-2 signed by the sponsor. If the sponsor is NOT able to physically appear with the dependent requesting the ID, the sponsor can: sign the DD1172-2 with a notary OR select the "Family ID Cards" to sign the required DD1172-2. This form is only good for 90 days. ID Card Office Online.
- A Family Member may provide a suitable, unexpired Power of Attorney to sign a DD 1172-2 in the sponsor’s stead. Family members must bring their old family member USID card and family members over 18 must bring two forms of current government issued ID.
Family Members Aged 21-23 Enrolled as Full-time Students - In addition to the requirements for family members above, the student must bring a letter from the university registrar’s office with a seal and signature stating the student is enrolled full-time. Dependent/institution MUST include the expected graduation date.
Qualifying Former Spouses/Unremarried Former Spouses will sign a DD 1172-2 as their own Sponsor at the DEERS facility and must also bring all other documents noted for family members above. Contact marya.r.samudio2.civ@army.mil to coordinate prior to the appointment.
OCONUS Contractors must bring their Letter of Authorization (LOA) and two forms of government issued ID.

Family Members

For military service members, first provide copies of the below required paperwork to your ADMIN section. For military service members, retired service members, 100% disabled veterans, or other designated beneficiaries, you can add an unmarried child, stepchild, adopted child, or ward to DEERS. Members must provide a birth certificate, certified adoption certificate, or court order of guardianship to add a child to DEERS. A social security card number is required. Newborns must be added to DEERS no later than 90 days after the birth of the child.

For military service members, first provide copies of the below required paperwork to your ADMIN section. For military service members, retired service members, 100% disabled veterans, or other designated beneficiaries, you must bring your sponsor ID, certified marriage certificate, and a copy of your spouse’s valid ID, Birth Certificate and social security card. All documents MUST be original or certified (in English).

For military service members, first provide copies of the below required paperwork to your ADMIN section. For military service members, retired service members, 100% disabled veterans, or other designated beneficiaries, you must bring a certified divorce decree. Your former spouse and any stepchildren not adopted by the sponsor will be removed from DEERS and Tricare eligibility. Turn in your former spouse’s and associated stepchildren’s dependent ID card(s) or advise them to do so.

Parents/parents-in-law, wards of the court, or unmarried children over 21 incapable of self-support may be added to DEERS and eligible for Tricare based upon a favorable determination from Defense Financing and Accounting Services (DFAS). If DFAS awards the case with a favorable determination, they will provide a letter for DEERS to enroll your family member. The family member must bring a DD 1172-2 signed by the sponsor and a DEERS official within the last 90 days, the favorable determination letter from DFAS, and two forms of unexpired government issued ID. Uploading Orders/ Change in Orders

First, the member should verify with the ADMIN section that your orders have been processed from DAMPS into IPPS-A. This step must be completed prior to the active-duty segment feeding into DEERS – this establishes TRICARE eligibility. This can take anywhere from 48-72 hours at a minimum after IPPS-A has been updated. DEERS personnel cannot update active duty status, the unit ADMIN must accomplish this.

IMPORTANT: Every time a Reservist or National Guard member receives a change in orders, they should verify TRICARE eligibility for the ENTIRE family (online) or call Tricare Customer Service at 888-874-9378. It may take up to 24 hours after the DEERS update for TRICARE to see a change in you or your family members’ status.

Dependent & College

Retiring & Records

TRICARE/ Dental/ Vision/ Prescriptions

The DEERS/ID office does not handle most TRICARE related questions. DEERS personnel CAN assist with eligibility questions.

Schedule an appointment to update the disability status.
If this is the first visit to a DEERS facility since receiving disability status, must bring a letter from the VA stating entitlement for 100% disabled along with their DD214 showing "Honorable" or "General under Honorable" discharge. 100% disabled veterans must bring their old USID card AND two forms of current government issued ID.
